The amount you pay for your medical or medical/dental coverage depends on which plan and what level of coverage you choose. Premiums are calculated on 24 equal pay deductions. Open enrollment occurs annually in the fall with new benefits effective on January 1. The Anthem Website www.anthem.com provides an extensive amount of information and provides those enrolled in the medical or vision plan to view claims and order ID cards.

Employees who enroll in a high deductible health plan will also have a health savings account (HSA) through The HSA Authority/Old National Bank. The brochure below contains all information associated with an HSA along with instructions on opening your online account with Old National Bank.
Employees who elect to contribute additional pre-tax dollars in their HSA accounts can start, stop or change your payroll deduction at any time throughout the year. Please complete the attached form and send to the Benefits Coordinator. Employees must personally keep track of your account balance to be sure you do not go over the allowable contribution amounts set be the government annually.
