School Required Immunizations

  • HEPATITIS A-NEW REQUIREMENT FOR ALL GRADES 2020-21 School Year. 
    All students will need proof of 2 doses of Hepatitis A, this change will most likely impact students in grades 8-12 as it is already a requirement for grades K-7. Proof of vaccination to Hepatitis A requires two doses at least 6 months apart.  If you plan to submit a religious objection to this for the year, please indicate which vaccines you object to or if your child can not have this vaccine due to medical reasons, have your physician complete the medical exemption form.

    To meet the yearly state requirements for vaccinations you must submit one of the following to the school nurse before the first day of school:
    1. A current vaccination record stating the vaccines and the datSavees for those that are required. Please do not resend a copy if the nurse has the most current copy.
    2. Religious Objection FORM & Notice of Exclusion for Objection print out the Religious Objection form and the Notice of Exclusion for Objection form make sure you select those vaccines that your objecting to on religious basis.  The form must be renewed every year.
    3. Medical Exception FORM & Notice of Exclusion for medical exception print out the Medical Exception form and have it completed and signed by your child's physician. Print out the Notice of Exclusion for Medical Exception form sign and send to the nurse with the physician signed medical exception form. 
    What if my doctors appointment is after the first day of school? 
      Please send a note with your childs name, date of birth and the date of the planned appointment to the school nurse before the first day of school. 
     
    Would you like a copy of the immunization records listed in the Children and Hoosier Immunization Registry Program (CHIRP)? 
    Contact your physician to provide you parental access to the registry.  See MY VAX  Please note school nurses can view the vaccinations but can not give you parental access this must be completed by your child's physican.
