School Required Immunizations

    NOTE: THERE ARE NEW VACCINE REQUIREMENTS FOR 6 GRADE and 12th GRADE STUDENTS FOR THE 2019-20 SCHOOL YEAR 
     
     
    Your students immunizations record must be up to date according to the grade requirements and a copy your child's vaccine updated record given to the school nurse before the first day of school AUGUST 6th, 2019. 
     
    If you would like a copy of your childs immunization records, please contact your physician to provide you parental access to Children and Hoosier Immunization Registry Program (CHIRP).
     
    Students who do not have information up to date will not be able to attend field trips or participate extra currular activities until this requirement has been completed.   
     
    If your physician's appointment is scheduled after August 6th, send a note with your student's name, student's date of birth and the date of the scheduled appointment to the school nurse in order to track compliance.

    RELIGIOUS OBJECTION OR MEDICAL EXEMPTIONS must be renewed every school year with the school nurse before the first day of school in order to be considered up to date.
    Please note for objections/exemptions, your student maybe subject to exclusion from school during an outbreak of a vaccine preventable disease under the guidelines of the Indiana State Department of Health and/or the Hamilton County Health Department.     If your student's school nurse has the most current immunization record and your student is up to date, do not send in a duplicate copy to the clinic.
